about Concierge Care





Who needs Concierge Care?



  • If you do not have Medical Insurance due to cost or other reasons.
  • If you tried to get Obama Care, TennCare, Medicare or Private Insurance but couldn't.
  • If you were dropped from Tenncare because of your modest income or other reasons.
  • If you were dropped from your parents' insurance because of your age or other reasons.
  • If you have a very high deductible in your current Insurance that you can never meet in a year.

Then, Concierge Care is for you!



How does Concierge Care work?



Concierge Care let's you see Dr. Ozoigbo once every month or multiple times a month up to a maximum 12times per year simply by signing up for one of the three choices below: -

  • (A) Basic - $150 per month + Processing fee = Office Visit with No labs or IM Injections or IV treatment/fluids - these can be added on for some fees.
  • (B) Deluxe - $200 per month + Processing fee = Office Visit with IM Injections or IV treatments/fluids and office diagnostics tests, but without ***Basic Labs - can be add on for some fees.
  • (C) Premium - $250 per month + Processing Fee = Office Visit with All inclusive including Office Labs/Diagnostics, Outside ***Basic Labs and Office IM Injections/IVs.

You can start Concierge Care today for yourself or your family by clicking on your choice of Concierge Care or at the Concierge Payments is the next sections below.


You can choose: -

  • (A) Basic - $150 per month (+ $5 Processing Fee),
  • (B) Deluxe - $200 per month (+ $7 Processing Fee)
  • (C) Premium - $250 per month (+ $8.50 Processing fee)

BASIC CONCIERGE CARE


Includes: Office Visit with calling in of your prescriptions to your pharmacy.

Good For: Patients on ADHD/ADD medications and other routine medications but are generally not ill.

Does Not Include: IM shots, IV Treatments/Fluids, Nebulizer breathing Treatments, Office Diagnostic Test, Outside ***Basic Blood or Other Lab Tests, Minor Office Surgical Procedures - These can be added on at extra fee.


DELUXE CONCIERGE CARE


Includes: Office Visit with calling in of your prescriptions to your pharmacy, IM Shot, IV Treatments/Fluids, Nebulizer Breathing Treatments, Office Diagnostic Tests,

Good For: Patients with Recurrent Pains requiring IM shots. Patients with recurrent acute exacerbations of their Chronic Diseases like Asthma, COPD, Hypertension, Herpes, Osteoarthritis, Rheumatoid Arthritis, Lupus.

Does Not Include: Outside ****Basic Blood or Other Lab Tests, Minor Office Surgical Procedures - These can be added on at extra fees.


PREMIUM CONCIERGE CARE


Includes: Office Visit with Calling in of your medications to your pharmacy, IM Shots, IV Treatments/Fluids, Nebulizer Breathing Treatments, Office Diagnostic Tests, Outside ***Basic Blood or Other Lab Tests.

Good For: Patients with multiple Chronic Diseases that require constant blood tests and other complicated diseases that require constant changing of medications. Difficult or Uncontrolled HTN, Diabetes, CHF or other Chronic Diseases.

Does Not Include: Outside Other Lab Tests, Minor Office Surgical Procedures - These can be added on at extra fees.
